The Surplus Lines application allows insurance agents, agencies, Risk Retention Groups and Risk Purchasing Groups doing business in Ohio to file their Department of Insurance (ODI)-required surplus lines tax data online.

Only insurance agents, agencies, Risk Retention Groups and Risk Purchasing Groups filing their surplus lines tax data should access this application.

Do You Need An Account?
ODI approves and creates accounts for this application. There is no other way to get an account for this application. In order to receive a new account you must meet the following criteria:
- You are or you represent a licensed insurance agent or agency; or a registered Risk Retention Group or Risk Purchasing Group; that does business in Ohio.
- You are required to file surplus lines or non-admitted premium taxes in Ohio.
- If you misplaced or forgot your user id and/or password information, contact the ODI at surplus.lines@insurance.ohio.gov.
