The Medical Liability Data Collection Application allows insurance companies, Risk Retention Groups and self-insurers to file their Department of Insurance (ODI)-required medical liability closed claims data online. 

Who Should Use This Application

Only those required to file medical liability closed claims data should access this application. Refer to Ohio Revised Code 3929.302 and Ohio Administrative Code 3901-1-64.
