Ohio Department of Insurance and Ohio Association of Health Plans Create Standardized Health Insurance Application


Uniform Small Group Application makes process easier for everyone


 

COLUMBUS - The Ohio Department of Insurance (the Department) and the Ohio Association of Health Plans (OAHP) today launched a new form, App1210, also known as the Ohio Uniform Small Group Application.  The Department and the OAHP have worked together to create a standardized small group employee application form in order to simplify the health insurance application process for employers seeking quotes from multiple carriers, making it easier for insurance agents, and especially employers and employees who will only have to fill out the form once.

“This collaboration with our industry partners will benefit employers, employees and agents alike by creating a singular application form,” said the Ohio Department of Insurance.

“We are pleased to be able to help streamline the process of applying for health insurance coverage in Ohio by creating a standardized form that many health insurance companies will use,” said Kelly McGivern, President and CEO of the Ohio Association of Health Plans.

Ohio health insurers who are participating in using the form are Aetna, AultCare, HealthAmerica, Medical Mutual of Ohio, Paramount, SummaCare/Summa Insurance Company, The Health Plan and UnitedHealthcare.

The Ohio Uniform Small Group Application form may be used by all Ohio licensed carriers or Health Insuring Corporations (sometimes known as HMOs) and does not need to be filed individually by each company with the Department.  Additionally, the form is in an electronic format which can be filled out and saved by the employee for future use.   This will allow the information to be sent through a secure email attachment to ease the use by all involved in the application process. For those that wish to have a hard copy, those will be available through agents and the insurers, if requested, or can simply be downloaded from the internet. 

OAHP will collect suggested changes as the form is used and will make necessary adjustments to the form for the Department’s approval in the future.

“This is the result of an ongoing process to find solutions that create efficiencies within our health care system,” added McGivern.

“This will be a valuable tool for health insurance consumers, employers and insurance agents,” said the Ohio Department of Insurance. “We are pleased that our industry partners worked with us to create a process that will make it easier for folks to navigate their health insurance options."

The Ohio Association of Health Plans (OAHP) represents 20 member health plans providing health insurance coverage to more than 7.5 million Ohioans.  Ohio’s health plans include commercial insurers, Medicaid Care Coordination Plans and Medicare Advantage Plans.
