Public Records Request

If you would like to request public records from the Ohio Department of Insurance (ODI), please visit the website and complete the electronic form. Note - A requester of public records does not have to make a written request, does not have to provide his or her identity and does not have to provide the intended use of the requested public records. The department may charge $.10 per page for copying records and there might be cases where some records are not available due to state retention schedules. If you prefer not to make a written request or if you have any questions, please contact the department at 614-728-1384.
 

Public Access Life and Health Insurance Filings

Insurance company Life and Health filings submitted electronically to the Ohio Department of Insurance are available online through the SERFF Filing Access (SFA)​ website at no cost.  The SFA website allows users to search for and download filings.  The SFA website also allows public comment on the health filings in order to maintain compliance with federal law. 


It is important to note all filings and all comments submitted are generally subject to Ohio's public records statute.


The Ohio Department of Insurance reviews all filings to consider whether the forms and rates comply with Ohio Insurance law, and to determine that the proposed rates are not excessive, inadequate, or unfairly discriminatory.

Effective September 1, 2011, health insurance companies are required to provide information to consumers when they file rate increases above a specified threshold defined by Federal regulators for individual or small group policies. The threshold is currently set at 10 percent, but will change to 15 percent for plan year 2019 and may change in future years. Disclosures for Ohio rate increases above the threshold are available on the Healthcare.gov​ website.
