Applicants must contact Prometric to make a reservation for their examination. Prometric will provide applicants with a confirmation number once the examination date and location are confirmed. The confirmation number should be written down and used for any contact with Prometric.  For more information refer to the Ohio Licensing Information Bulletin.

Applicants may make a reservation to take an examination by one of the following methods:

Online: Go to www.prometric.com/ohio to make an online reservation for an examination. Online reservations must be made at least 24 hours before the desired exam date.

By Phone: Contact Prometric at 1-877-346-4014 at least 24 hours before the desired exam date. The customer care hours are Monday – Friday 8:00 am – 9:00 pm (EST)      

By Fax: Complete the Fax Reservation form, found on page 21 of the Ohio Handbook and fax to 1-877-341-9469. Fax lines are available 24 hours a day, seven days a week. The completed form should be faxed at least 4 business days before the desired examination date. A confirmation of the reservation will be returned by fax within 24 hours of receipt of faxed request.

• Legal name, address, SSN, day time telephone, and date of birth

• A failing score report (if retaking an examination)

Fees:
Fees must be paid at the time of reservation by credit card (Master Card or Visa), debit card, voucher, or electric check. Fees will not be accepted at the test center.
