Any business entity (corporations, limited liability companies, limited liability partnerships, general partnerships) conducting the business of insurance in Ohio, must be licensed by the Ohio Department of Insurance.
The Department issues limited authority licenses to Business Entities for the following lines of insurance:
- Credit insurance products
- Crop
- Funeral Expense
- Portable Electronics (Do not use the Uniform application. Use the specific Portable Electronics application)
- Rental car insurance
- Travel insurance
NAIC UNIFORM BUSINESS ENTITY APPLICATIONS
The business entity must have at least one Ohio licensed insurance agent on the corporate structure that will be responsible for the business entities compliance with Ohio’s insurance laws.

INFORMATION FOR ALL APPLICANTS
If the required applications or attachments are omitted or incomplete, the application will not be processed and will be returned to the business entity at the address on the application.
An appointment is not required at time of application, but is required in order to sell, solicit, or negotiate policies/information for a company.
